There is no charge for a passport that is less than one year old. You can mail your papers to any shipping company. The expedited service fee will be charged to applicants who choose it.

More than a year

Various forms will be required for applicants with older passports than one year. The same documents will be required and fees will apply. These papers will be required to replace your original.

  • Completed form DS-82
  • Your valid passport
  • An original document showing name change such as:
    • Certificate of Marriage
    • Divorce decree
    • Order of the Court
    • Adoption certificate
  • A passport size photo
  • Application renewal fee

You can either mail your application or go in person.

Applicants who have a passport with a validity of more than 15 years cannot apply for either Form DS-5544 or Form DS-82. These documents are required if you fall into this category:

  • The original document that shows the name change such as:
    • Certificate of marriage
    • Divorce decree
    • Order of the Court
    • Adoption certificate
  • A passport size photo

Applying for a Passport for the First Time

These instructions will help you apply if you don’t have a passport but have had a name change. It is necessary to apply in person, not by mail. You will need to apply in person for either an adult or minor. These documents must be submitted:

  • Completed Form DS-11
  • Documentation proving U.S. citizenship
  • Photocopy of your current ID and a new name
  • One passport size photo
  • Application fee
